How to Choose a Surrogacy Agency

There is no single agency model that is right for every intended parent. These six factors give you a practical framework for narrowing a long list down to a smaller group worth interviewing.

1

Agency Size and Operating Style

A large organization may appeal to someone who values extensive infrastructure and specialized internal departments, while another family may prefer the closer relationships associated with a boutique agency.

Large or corporate-scale agencies may have dedicated professionals handling matching, insurance, escrow coordination, psychological support, and international cases. Processes may be more standardized, and a larger recruitment operation may support greater geographic reach.

Mid-size agencies may offer many of the same resources while maintaining a more concentrated team.

Boutique agencies place greater emphasis on direct contact with a smaller number of coordinators and a more individualized client relationship, and usually complete the fewest journeys in a year.

The relevant question is not "Is this agency big or small?" It is "Which operating model gives us the type of communication, resources, and experience we want throughout our journey?"

During agency consultations, intended parents can ask who will manage their case, how many people will be involved, what happens after normal business hours, and which services are performed internally versus outsourced. Size labels are practical classifications based on publicly visible operating footprint and service model — not official industry categories, and not a measure of quality.

2

Published Success Rate — and How It's Defined

A success rate can be one of the first statistics an intended parent notices, and one of the easiest to misunderstand. The key issue is the denominator. Ask:

  • What does the agency define as a successful journey?

  • What time period does the statistic cover?

  • Does it represent live births, ongoing pregnancies, or another outcome?

  • Which intended parents or journeys are included — everyone who signs, only those with embryos, or only those completing a transfer?

  • Has the methodology changed over time?

The percentage can be useful, but the definition behind it is equally important. Treat the number as the beginning of the conversation rather than the end of it.

3

Program Type:
Fixed or Variable

A lower initial estimate does not necessarily mean a lower total journey cost, while a higher fixed-cost program may provide additional financial protection if a transfer fails, a rematch is required, or another unexpected event occurs. Some agencies offer both structures, letting intended parents choose between predictability and a lower entry price.

4

What the Program Actually Protects Against

Not all surrogacy cost programs are created the same. Check specifically whether a program covers additional embryo transfers, surrogate rematching, repeat screening, insurance, legal expenses, surrogate compensation, travel and lost wages, refunds if a baby is not born, and fertility-clinic or IVF expenses. Our surrogacy costs guide breaks down what's typically included and excluded.

5

Published Surrogate Return Rate

A returning surrogate has already completed at least one journey and chooses to participate again — which can say something about the surrogate side of the agency experience. But definitions vary: calculations may include sibling journeys, return journeys, referrals, or simply stated interest in returning. Pay attention to the agency's exact language.

What the Agency Is Best Known For

Agencies often provide many of the same core services, so broad claims such as "personalized service" or "full support" do little to distinguish one from another. Look instead for concrete differentiators: a large full-service infrastructure, a boutique operating model, fixed-cost financial protection, fast published matching timelines, physician involvement, staff with personal surrogacy experience, specialized international experience, a distinctive matching model, or longstanding LGBTQIA+ family-building experience.

Questions to ask during a consultation
Who will manage our case, and how many people will be involved? What happens after normal business hours? Which services are performed internally versus outsourced? Exactly how is your published success rate calculated? What does the program exclude?

Because surrogacy programs, pricing, and published statistics change, intended parents should always confirm current terms directly with an agency before signing an agreement. See our methodology for how we compare agencies.
